A request for judicial notice must be made in writing and in a separate document listing the specific items for which notice is requested and must comply with CRC 3.1306(c) (amended eff 1/1/17). CRC 3.1113(l) (amended eff 1/1/17)
The caption of any pleading subsequent to the original complaint may bear the short caption of the case, identifying just the first party on each side. CCP § 422.40
The first page of the pleading must specify, immediately below the number of the case, the date, time and location of the hearing and the name of the hearing judge, the title of any attached document other than an exhibit, the date the action was filed and the trial date. All documents filed in cases subject to civil case management, with the exception of the original summons and original civil complaint, must have clearly typed on the face page of each document, under the case number, the following:

A request for judicial notice under Evidence Code Sections 452 or 453 must provide the court and each party with a copy of the material of which judicial notice is requested. CRC 3.1306(c) (amended eff 1/1/17)
If the material of which judicial notice is requested is part of a court file in the same court, the party must specify what part of the file is requested to be noticed, and make arrangements with the court clerk to have the file in the courtroom at the time of the hearing.
